You may have heard us talk a lot about person-centred care at Oaklands. But we wanted to share why person-centred care is so important to us, and how it really makes a difference to the residents in our care.
What is person-centred care?
Person-centred care prioritises the individual receiving care and places them at the heart of all decision making.
At Oaklands, we take a collaborative approach and involve each resident and their family from the beginning, to understand their personal preferences and requirements, to make sure we can fully meet their needs.
Individual care plans
Our wonderful team creates an electronic care plan that includes important information about our residents’ nutrition, hydration, continence and medication. It can include simple things like their favourite food and drink, their likes and dislikes, interests and what time they like to get up and do things, so we can deliver outstanding quality care.

The most important thing to us is being able to meet the needs of our residents, so we can provide the highest levels of care and boost our residents’ well-being.
We are passionate about making sure our residents are treated respectfully and their choice, dignity and best interests are acted upon at all times.
Information easily accessible
Our care plans play an important part in being able to deliver the right person-centred care. It means anyone new to the team has information on all aspects of our residents and this can be communicated well to healthcare professionals.
Boosts residents’ well-being
When individuals feel respected, heard, and involved in their care, they are more likely to have a positive experience.
Tailoring care to individual needs and preferences makes it easier to stick to treatment plans. For instance, one resident might only take their medicine with orange squash and not water, knowing this detail will mean they happily take their medicine, which will improve their overall health and well-being.
Our incredible team plans tailored activities based on our residents’ personal preferences and creates a calendar for the whole month, with fun and engaging activities that are inclusive for everyone.
One of our residents’ favourite activities is our ‘Send a Smile Campaign’ where we make their individual wishes come true.
