Person Centred Approach

Here at Oaklands Rest Home we believe in involving and tailoring a person’s care to their interests, abilities, history and personality. This helps them to take part in the things they enjoy and can be an effective way of preventing and managing behavioural and psychological symptoms of dementia.
With a dedicated Lifestyle Co-ordinator all Service Users can have a tailor made activity plan to best suit their needs with many of our activities are aimed towards dementia as it can be seen that these activities can in some instances pro long the loss of memory.

Oaklands Rest Home is a dementia registered residential care home that is equipped to deal with the many complex needs that arise from having Dementia. It is situated in the attractive and tranquil settings of the New Forest National Park.

Our Ethos

• To deliver a service of the highest quality that will improve and sustain the service user’s overall quality of life.

• To ensure that the care service is delivered flexibly, attentively and in a non-discriminatory fashion while respecting each service user’s right to independence, privacy, dignity, fulfilment, and the rights to make informed choices and to take risks.

• To ensure that each service user’s needs and values are respected in matters of religion, culture, race or ethnic origin, sexuality and sexual orientation, political affiliation, marital status, parenthood and disabilities or impairments.

• To ensure that the care service in whole is delivered in accordance with the Statement of Terms and Conditions agreed at the point of moving into the home.

• To manage and implement a formal programme of staff planning, selection, recruitment, training and personal development to enable service user care needs to be met.

• To manage the care service efficiently and effectively to make best use of resources and to maximise value for money for the service user.

• To ensure that all service users receive written information on the home’s procedure for handling complaints, comments and compliments, and how to use it.

Lifestyle Co-ordinator

At Oaklands Rest Home we have a lifestyle co-ordinator that tailors an activities program for each service user. A key aspect of coordinating our activities is to ensure there is variety in the programme of daily events. We gain lots of information about what type of things residents might be interested in during reminiscing sessions and through their relatives. At Oakland’s Rest Home we embrace technology to ensure we “Put The Person At The Centre Of Care” .

Reminiscing

One way we use technology to ensure we have a person centred approach to our care is by Using Remind Me Care Software.

Dementia and care of the elderly is demanding; for the person being cared for and for their carers. But the better a carer knows the person, the better it is for everyone. RemindMeCare places the person at the centre and provides unique tools and activities that assist them be better known for who they truly are.

Much work has been done regarding person centred care delivery, the power of reminiscence therapy and different methodologies for dementia care. Opinions vary but all agree on one thing, that Life Story knowledge brings benefits to all. Until now it’s not been possible to easily use internet content to explore a person’s Life Story and likes and dislikes other than on a generic basis. ReMe changes that …

Relatives can login to the system offering details to the life story and with suggestions of stimulus which will interest their loved ones, as it is important to think about the specific interests of the individual residents living in the care home. The system can then search the internet for pictures, music and videos all relating to that persons profile engaging with Resident’s who before have struggled for a connection.

Activities

There is a range of Activities available to our residents at Oakland’s. The Lifestyle Co-ordinator provides a number of these activities daily and the Planner for the Month can be viewed on the EVENTS PAGE.

From helping feed and look after the chickens, to Arts & Crafts our resident’s can participate as much or as little as they so wish.

Dementia Care

Oaklands specialises in residential and dementia care and has been thoughtfully designed with dementia care specifically in mind.

Personal Care

Personal care (sometimes referred to as residential care) is care that is provided by trained carers rather than nurses. As well as being specialists in dementia care, our staff are also trained to provide personal care for those who need assistance with what we term the activities of daily living; for example washing, dressing, eating and mobility.

Respite Care

Short break or respite care is available for residents who require care for a temporary period. We offer respite care for just a few night’s stay or for up to several weeks at a time.

Day Care

Oaklands offers Day care, which is available to residents who just want to stay with us during the daytime and not for an overnight stay.
