Discharge to Assess (D2A)
The team here at Oaklands work closely with Hampshire County Council and West Hampshire CCG to provide support with Discharge to Assess (D2A).
What’s D2A?
Sometimes an elderly loved one can be ready to leave hospital but will need to undergo an assessment to establish their on-going health and social care needs first. However, assessing patients and arranging the right support to help them return home safely often takes time.
For older people in particular, we know longer stays in hospital can lead to worse health outcomes and can increase their long-term care needs. In these circumstances, an elderly patient will be discharged through the Discharge to Assess (D2A) pathway into a care home like Oaklands Rest Home for a temporary period.
